Critical Incident Review

  • Review the incident and the action plan, validate the root cause, assess policies and procedures, and interview all applicable front-line staff and patient and family advisors. 
  • Process observation over multiple shifts will be used to identify gaps between expected and actual practice state. 
  • Special attention will be paid to Transfer of Accountability, Documentation and Communication, and Identification and Response to Clinical Deterioration. 
  • Unit morale and leadership as drivers of patient safety, will also be assessed.
